Revive Me Weight Loss Program
We have streamlined our process into three simple steps to ensure our clients receive the best care possible:
Consultation with a Provider
Clients begin by filling out a comprehensive medical intake form to provide their medical history. Our dedicated nurse reviews this information to determine if they are a suitable candidate for our program. If deemed appropriate, the client is then directed to a preferred facility for necessary lab work (CMP, A1C, & TSH).
Medical Assessment
Once the lab results are obtained and sent to us, we schedule an initial telehealth consultation with our medical provider. During this consultation, the provider assesses the client's health and discusses potential medication options.
Medication Delivery
If approved for the program, the client will be contacted by our nurse to confirm the medication choice. We then send a prescription to our partnered pharmacy, and the medication along with necessary supplies are shipped directly to the client's door.
